Form Elements Overview

Form elements are the building blocks of your feedback form. Each element type serves a specific purpose in collecting user feedback.

Element types

Use the sidebar or the links below to open the doc for each form element:

• Welcome Screen — Introductory screen before the survey
• End Screen — Closing screen with thank-you or next steps
• NPS — Net Promoter Score (0–10) question
• Single Choice — One option selected from a list
• Multiple Choice — One or more options selected from a list
• Rating — Star or numeric rating scale
• Short Answer — Single-line text response
• Long Answer — Multi-line text response
